While rare, some horses may deliver twin foal babies.  Many vets says that a horse is not designed to successfully carry twin fetus through pregnancy, and many time one of the twin horse foals dies and is re-absorbed by the mare.

As noted, is is not uncommon for one twin horse foal to be significantly smaller than their twin.  According to this article, the smaller twin foal is commonly due to spatial and nutritional limitations of the mare.
